ALL artists must have current first aid, CPR, blood borne pathogen certs and a current DPOR license – NO EXCEPTIONS!  Bring all of Your Valid Documents to the Event

*IF A TATTOOER HAS PREVIOUSLY HELD A TATTOOER OR GUEST TATTOOER LICENSE IN VA, BUT DOES NOT HAVE THOSE RECORDS TO FILL IN THE INFORMATION FOR ITEM NUMBER 11, please call the DPOR and ask them to look it up: 804.367.8509

*LEAVE THE “GUEST SPONSOR PARLOR/SALON – RESPONSIBLE MANAGER” SECTION BLANK, AT THE VERY BOTTOM OF THE FORM.

Guest tattooer application, credit card payment form, and certifications must be faxed or mailed. If you’re going to mail it, please do so priority or overnight.

DOWNLOAD “EASE FAX” or “GENIUS FAX” TO YOUR DEVICE: it’s pretty necessary for the VA DPOR, easy, and cheap (pay per fax, no monthly subscriptions), or you can find someplace to fax from, like FedEX, UPS, or a library.
